Free Diabetes Education Materials for Patients and Professionals

publications & resources Each year, the Texas Diabetes Council stocks free diabetes education materials available by request through the Texas Department of State Health Services warehouse.

In order to continue this valuable service, please order quantities that you know you will be using in one-on-one patient education or for your own learning purposes. The Texas Diabetes Council wants everyone who can benefit from these materials to receive copies; however, many, such as "Taking Charge of Your Health," should not be distributed in large quantities at health fairs and other events targeting the general public. Simple, less expensive brochures, such as "Could You Have Diabetes?" are more suitable for general diabetes awareness.

Use the descriptions below to determine which piece is best suited for your educational purpose, and order as needed.

Ordering Instructions:

1. Order online using the Texas Department of State Health Services Forms and Literature Inquiry and Order Entry System.

If you do not have internet access or are having difficulty ordering online, submit a request in writing (below).

2. Submit a request in writing or through a Texas Department of Health Form No. AG-30 (see page 2 of literature catalog) to the Texas Department of Health Warehouse. Written requests should contain the following information:

  • Requestor's Name
  • Shipping Address (For larger orders, please provide a street address, NOT a post office box.
  • Organization, Affiliation or Professional Status (If applicable)
  • Catalog number(s) and title(s) of items requested
  • Quantity (Most items limited to a maximum of 100 copies per order)
  • Date requested
